Content on the Site is provided for general information only and is not medical advice, diagnosis, or treatment. It does not create a provider-patient relationship. 2. Appointments and Communications

Appointment requests submitted through the Site are requests only and are not confirmed until our staff contacts you. Message and data rates may apply to SMS communications; see our SMS Terms for details. Do not use Site forms for urgent medical matters.

Your use of the Site is also governed by our Privacy Policy, which describes how we collect and use information. Protected health information is handled in accordance with applicable law, including HIPAA where it applies.
